Complete Decongestive Therapy (CDT)—also called complex decongestive therapy—is the guideline-style treatment package for lymphedema, not a longevity add-on. It combines manual lymphatic drainage, multilayer compression (bandaging then garments), meticulous skin care, and decongestive exercise, usually in two phases: intensive volume reduction, then lifelong maintenance. A certified lymphedema therapist (CLT) is the point—not a med-spa “lymphatic hour.”
Why it is not a menu drip
Lymphedema is protein-rich swelling from damaged or overloaded lymphatics (cancer treatment, congenital, obesity-related, infection). CDT treats that. A NormaTec nap after leg day is recovery compression. Mixing the two lets clinics charge CDT prices for a massage. ISL consensus and oncology groups describe CDT as multimodal; skipping garments after “just MLD” is how volume returns.
Limits
CDT does not cure every lymphatic malformation. It requires adherence. Contraindications overlap MLD (DVT, untreated infection, unstable CHF). Cancer patients need oncology context. Insurance often wants a diagnosis and referral. If a lounge offers “CDT” without bandaging, garments, or a CLT, it is not CDT.
Referral notes, surgical/radiation history, current garments, and photos of swelling if you have them. Do not start a ‘CDT intensive’ the week of an untreated cellulitis.
Bandages are bulky. Plan clothes and driving. Report fever or rapidly worsening swelling.
Skin breakdown under poor bandaging, missed infection, cardiac fluid shift, and false security if only the massage portion is delivered.
