What Is Near-Infrared Light Therapy?

Near-infrared (NIR) light therapy is PBM at wavelengths you barely see—typically ~800–880 nm (810 and 850 nm are clinic favorites). It is sold for joints, muscle, and “brain helmets” because NIR penetrates deeper than 630–670 nm red. It is not an infrared sauna: saunas dump heat; research PBM uses modest irradiance without cooking the room. See red light vs infrared and infrared sauna.
How clinics mix it
Most panels fire red and NIR diodes together. That is convenient and makes it hard to know which wavelength did what. Helmets and pads aimed at knees or the skull are often NIR-heavy. Dose still depends on irradiance at the skin (or scalp), time, and distance—not the word “near-infrared” on the brochure.
Evidence and limits
Deeper penetration is physics, not a guarantee of better outcomes. Musculoskeletal PBM studies are heterogeneous. Transcranial NIR for cognition or depression is investigational, not a dementia treatment. If the device is warm like a heating pad, you may be getting photothermal effect, which is a different claim. Eye safety still applies; NIR is invisible, so you will not blink it away.
Bare skin or thin fabric per device instructions. Remove photosensitizing confusion with your med list. Do not stare into emitters.
No downtime. Transient warmth is OK; pain or visual spots are not.
Retinal exposure, skin irritation, overheating if the “NIR” unit is actually a heater, and false confidence from brain-helmet marketing.
