Aesthetic biostimulators are injectable products used to produce gradual tissue or contour change through material support and a biologic response. Common examples include poly-L-lactic acid (PLLA, such as Sculptra®) and calcium hydroxylapatite (CaHA, such as Radiesse®). Product labeling, approved areas, dilution, injection plane, and expected effect differ.
Not conventional HA filler
Unlike hyaluronic-acid filler, PLLA and CaHA are not readily dissolved with hyaluronidase. Results develop over weeks to months, and overcorrection may be difficult to manage. “Collagen stimulation” does not mean unlimited tightening or predictable reversal of aging.
Risks are product- and technique-specific
Nodules, granulomas, asymmetry, inflammation, infection, and unwanted volume can appear early or late. Because these are injected materials, accidental vascular injection can cause necrosis and other severe injury; facial filler injections have also been associated with blindness and stroke.
Plan in stages
A conservative plan should define treatment area, product, vial or syringe count, dilution or reconstitution, injection plane, interval, and endpoint. Allow biologic response to develop before automatically adding more product.
Disclose all prior filler and biostimulator locations, implants, autoimmune or inflammatory disease, allergies, infection, dental work, anticoagulants, pregnancy, and tendency to form scars or nodules.
Bruising, swelling, tenderness, and palpable firmness can occur. Follow product-specific aftercare rather than generic massage instructions. Report blanching, severe pain, visual symptoms, fever, drainage, or persistent nodules promptly.
Bruising, swelling, pain, asymmetry, infection, nodules, granulomas, delayed inflammation, unwanted volume, and difficult correction. Intravascular injection can cause tissue necrosis and potentially visual or neurologic injury. These products are not simply reversible with hyaluronidase.
